Falls Prevention

In the UK 1/3 people over the age of 65 and half of people aged 80+ fall each year. 

Many are preventable. We can reduce the risk of falling through physical conditioning, home environment review/ changes, mobility equipment, driving medical and pharmacological review - and much more. 

Post Falls Confidence

Loss of confidence is often completely overlooked when considering why an adult may not be back to their usual selves following a fall. And reduced confidence increases the risk for further falls. 

We work with each individual to identify routes back to confidence and these can include physical conditioning, equipment aids, practice practice practice, coaching, strengths based motivational interviewing.

Rehab After Injury, Illness or Hospital Stay

We can work with you or your loved one to functionally rehabilitate after a long term stay in hospital, long term illness or an injury. Or even just after a period of deconditioning.  We focus on your goals, and help return to the activities and tasks that give you purpose.

Struggling to Live Independently

We believe that most people over the age of 50 have started limiting some of their activities due to real or perceived age issues. 

We can work with you to keep active and engaged in the activities you want to take part in. 

We work with you or your loved ones to facilitate a good death. We can facilitate important conversations between loved ones, help you and your loved ones prepare emotionally for death and we can support you to live as independently, as comfortably and as well as possible, for as long as you live.  

We can help you or your loved one stay at home and remain as independent as possible. Physical conditioning, task adaptation, adaptive equipment and devices, problem solving, finding joy in task performance and more.

Community Access

We can support you maintaining links outside of the home - to your family, church, friends or jut the local shops. Through retraining, advocacy, formal links, signposting and myriad creative ways.

Home Adaptations

Sometimes the tiniest of changes can have massive positive impacts. We can work with you and with our trusted partners to make changes within the home that enable to to live how you'd like to. This could be anything from an additional rail in just the right spot, or a whole new bathroom. We acknowledge that Social Services wait times in most London Boroughs are over 18 months and so can look at providing the more impactful or urgent changes while you await free-at-point of service support.

Deconditioning

We can help you feel fitter again. Research suggests most adults (of any fitness or age) aren't fully compliant with home physiotherapy exercises.  We can help you re-condition through meaningful, fun and functional activities - tailored to you, in your home, done with you.

Equipment and Technology Needs

We can help you consider the right equipment and technologies to support your goals. Using adaptive  equipment isn't about capitulating to functional changes, but about enhancing independence and utilising tools that enable us to thrive.

Manual Handling and Care Agency Support

If you are living with new ways of undertaking functional tasks - like being hoisted to and from bed/chair or receiving personal care in bed, we can conduct reviews of manual handling methods to support you. We can provide Manual Handling reports to care agencies and work with carers to understand your unique moving and handling needs.

Difficulty with NHS or Social Services

We can review the input you've had from NHS and Social Services and advocate for you: for more input, for better input, and for input with better outcomes. We can help navigate these services and we can speak their language to help get answers to questions. We can also case manage you or a loved one.

We can help you plan the future and can support and facilitate crucial conversations with family members., supporting in decision making around care, residential placement and much more.

Self-Funded Care: Personalising Care That's right For You

If you are living with new ways of undertaking functional tasks - like being hoisted to and from bed/chair or receiving personal care in bed, we can conduct reviews of manual handling methods to support you. We can provide Manual Handling reports to care agencies and work with carers to understand your unique moving and handling needs.   

We know that the ability to engage in life's tasks, activities and roles is integral to wellbeing and health and we work with clients and support networks on the path to wellbeing.  


We know that full participation in our own lives is integral to the wellbeing and health of the communities  and the systems we live within: families (chosen or blood), friendships, work-places, neighbourhoods and communities like religious, sporting or volunteer, sub-cultures and volunteer groups. 

We also know that we can't always support you with every challenge and so we also signpost to other providers, clinicians and/or services whenever there may be an opportunity to optimise outcomes.
